Core Responsibilities
The Clinical Pharmacist IE provides both distributive and clinical pharmacy services. Key duties include:
- Conducting medication reconciliation and patient assessments upon admission and discharge.
- Identifying and resolving drug-related problems through therapeutic monitoring.
- Collaborating with healthcare teams to determine optimal drug selection and dosages.
- Serving as a preceptor and mentor to pharmacy students and interns.
- Providing in-service education to nursing and medical staff on new therapies.
- Participating in on-call rotations to support 24/7 hospital pharmaceutical needs.
Qualifications & Requirements
Candidates must meet the following professional standards to be considered:
- Graduation from a recognized School of Pharmacy (BSc. Pharm or PharmD).
- Current licensure in good standing with the Newfoundland and Labrador Pharmacy Board.
- Membership with the Pharmacists’ Association of Newfoundland and Labrador (PANL).
- Demonstrated ability to establish effective interprofessional working relationships.
- Satisfactory record of work performance and attendance.
- Preferred: Completion of a hospital residency program and previous hospital experience.
