Role Responsibilities

As a Care Navigator, you are the face of the practice. Your primary duty is to ensure the facility is ready for patients daily and that all clinical rooms are fully operational. This role, featured on Government Jobz, involves high-level interaction and administrative precision.

Key Duties:

• Manage face-to-face and telephone patient inquiries efficiently.
• Handle home visit requests and coordinate with the District Nursing team.
• Book appointments using clinical systems (EMIS and Docman) and manage E-consults.
• Process new patient registrations, medical record transfers, and insurance reports.
• Maintain strict patient confidentiality and data privacy at all times.
• Provide chaperone support for clinical staff when required (training provided).
• Manage outgoing post, shredding, filing, and general reception upkeep.
• Conduct end-of-day security and kitchen safety checks.
