Shift Patterns
Varied working hours to ensure coverage:
- Standard: 9:00 AM – 5:00 PM
- Early: 6:00 AM – 2:00 PM
- Evening: 2:00 PM – 8:00 PM
About the Service: Via Rough Sleepers Projects
Via offers an integrated community service providing Intensive Recovery Outreach and Dedicated Support to rough sleepers in Camden and Islington. This CQC-registered project delivers wrap-around care to street-active individuals dealing with substance misuse, mental health issues, and trauma.
The service combines clinical interventions (such as OST and community detox) with psychosocial support to help individuals re-engage with mainstream health and housing services.
Your Responsibilities
As an Outreach Practitioner found via Government Jobz, you will:
- Conduct assertive street outreach to engage/re-engage homeless individuals.
- Deliver evidence-based psychosocial interventions and harm reduction advice.
- Conduct risk assessments and create person-centered care packages.
- Liaise with partner agencies (housing, GPs, mental health teams) to ensure holistic care.
- Monitor and support health interventions like BBV (Blood Borne Virus) screenings and Naloxone distribution.
